The HCC is responsible for advocating on behalf of individuals to assure their needs are met and Gatesway staff are providing the supports and services identified in Individual Plans. This includes supporting, encouraging, and enhancing individual’s self-respect, personal dignity, health and safety, providing ongoing training to individuals, coordinating all individual related activities and serving as lead staff member in assuring Habitation Training Specialist’s job duties and performance in these areas. For the purpose of documentation this position will be referred to as Health Care Coordinator (HCC) and is also known as the House Supervisor (HS).
4. The HS/HCC is accountable for assuring the safety, dignity, and confidentiality of all individuals. This includes maintaining service to the clients, working with other team members in the development and implementation of Individual Plan programs, maintaining an ongoing awareness and knowledge of Individual Plan goals and objectives, providing related training and interaction to support a meaningful day, achievement of outcomes, and coordinating emergency and evacuation drills. The HS/HCC is accountable for working a minimum of a 40 hour workweek (staffing patterns will vary based on individual needs) and will be available for emergency coverage during episodes of staff shortage as needed.
5. The HS/HCC is responsible for assisting the assigned Program Coordinator in checking and maintaining all Individual Plans and records, assuring assigned assessments and reviews related to Individual plan development are completed and implemented in a timely manner, and reporting any suspected abuse, neglect or exploitation of individuals served according to applicable law and agency policies. The HS/HCC is also responsible for completing when necessary physical intervention documentation and Accident/Incident Reports following established procedures, keeping the Program Coordinator and or the Lead Program Manager informed of staff and individual concerns or problems as they arise. The HS/HCC demonstrates a working knowledge of applicable rules, policies, procedures and contract standards. The HS/HCC assures time sheets, mileage claims, monthly reports and other specified documents are submitted for assigned staff in accordance-established time frames.
